“Back in the day, the majority of exercise studies focused on the parts of the body from the neck down, like the heart and lungs,” says Ozioma Okonkwo, assistant professor of medicine at the University of Wisconsin School of Medicine and Public Health. “But now we are finding that we need to go north, to the brain, to show the true benefits of a physically active lifestyle on an individual.”

Exercise might be a simple way for people to cut down their risk for memory loss and Alzheimer’s disease, even for those who are genetically at risk for the disease. In a June study published in the Journal of Alzheimer’s Disease, Okonkwo followed 93 adults who had at least one parent with Alzheimer’s disease, at least one gene linked to Alzheimer’s, or both. People in the study who spent at least 68 minutes a day doing moderate physical activity had better glucose metabolism—which signals a healthy brain—compared to people who did less.

The brain benefits of exercise go beyond disease prevention. Okonkwo has also shown that people who exercise have greater brain volume in areas of the brain associated with reasoning and executive function. “We’ve done a series of studies showing that increased aerobic capacity boosts brain structure, function and cognition ,” he says, “Other people have found exercise can improve mood.” Okonkwo’s research has also shown that exercise can diminish the impact of brain changes on cognition, not just prevent it. “Exercise is the full package,” he says.

Exercise likely improves brain health through a variety of ways. It makes the heart beat faster, which increases blood flow to the brain. This blood delivers oxygen—a good thing, since the brain is the biggest consumer of oxygen in the body. Physical activity also increases levels of brain-derived neurotrophic factor (BDNF), which i s known to help repair and protect brain cells from degeneration as well as help grow new brain cells and neurons, says Okonkwo .

In one study. Joe Northey, a PhD candidate at the University of Canberra Research Institute for Sport and Exercise in Australia, showed that when people ride a stationary bike, they experience increased blood flow to the brain, and within that blood are a range of growth factors that are responsible for cell growth and associated with improved brain function. “Considering exercise can also reduce the risks associated with common lifestyle diseases that impact the brain, such as high blood sugar and hypertension, it is further motivation to try to incorporate exercise as part of a healthy lifestyle,” says Northey.

Aerobic exercise, like running and swimming, appears to be best for brain health. That’s because it increases a person’s heart rate, “which means the body pumps more blood to the brain,” says Okonkwo. But strength training, like weight lifting, may also bring benefits to the brain by increasing heart rate. The link between resistance training and better brain health is not as established, but research in the area is growing.

For now, Northey recommends a combination of the two. “Combining both is ideal,” he says, for all of the other benefits exercise bestows on the body. “In addition to improving your brain function, you should expect to see improvements in cardiorespiratory fitness and muscle strength, as well as reducing the risk of obesity, diabetes and hypertension amongst other diseases.”

Exercise can prevent insulin resistance by prompting the body to remove damaged cells and enhancing the quality of mitochondria, the cell’s energy powerhouses.

Type 2 diabetes affects 4.5 million people living in the UK and 29 million people in the US.

The study also casts doubt on the previously held view that increasing the quantity of mitochondria could help fix some consequences of a high fat diet, including insulin resistance.

The researchers found that the benefits from physical activity were not affected by the quantity of mitochondria.

Lead researcher Megan Rosa-Caldwell, a doctoral student at the University of Arkansas, found that mice genetically engineered to have higher quantity of mitochondria were not more protected against high-fat diet induced insulin resistance.

How was the research conducted?

The researchers fed all the mice in the study a Western diet high in fat.

The genetically engineered and control mice were further divided into a group that was allowed to exercise, and a sedentary group.

Their results showed that physical activity, regardless of the amount of mitochondria, offered similar health benefits against insulin resistance.

The researchers said that it appears that exercise’s ability to help remove damaged cells and enhance the quality of the mitochondria may be more effective for preventing insulin resistance.

But they said these aspects need to be further tested.

Exercise offers ‘the greatest protection’

Ms Rosa-Caldwell said that with rates of obesity and type 2 diabetes continuing to increase, understanding the cellular processes that help or hurt insulin resistance can help doctors better tailor effective preventative measures such as exercise.

She added: ‘For now, physical activity is the greatest protection, but further research may enable us to prevent and treat insulin resistance, and subsequent diabetes, more effectively.’

The research was published in the journal Experimental Physiology.

Maintaining good exerciseregimen may be difficult and risky, especially for people with Type 1 diabetes, because exercise tends to cause fluctuations in blood sugar, which may even become dangerous, researchers warned.

Patients with Type 1 diabeteshave to monitor their blood glucose levels before, during and after exercise, said Michael Riddell, Professor at York University in Ontario, Canada.

Regular exercise can help individuals with diabetes to achieve their blood lipid, body composition, fitness and blood sugar goals.

However, for people living with Type 1 diabetes, the fear of hypoglycemia, loss of glycemic control, and inadequate knowledge around exercise management are major barriers, Riddell said.

In the study, published in the Lancet Diabetes & Endocrinology, a team of 21 international experts brought out a set of guidelines on glucose targets for safe and effective exercising as well as nutritional and insulin dose adjustments to prevent exercise-related fluctuations in blood sugar.

Diabetic patients should maintain a healthy body weight nor do they achieve the minimum required moderate-to-vigorous aerobic activity of150 minutes per week.

In paediatric patients, exercise reduces the cardiovascular disease risk profile, improves the sense of well-being and brings down average blood glucose levels, whereas in adults, being physically active may decrease the risk of both diabetic eye disease and kidney disease.

They also have a better chance of achieving target levels of glycated hemoglobin levels, blood pressure levels and a healthier body mass index (BMI) when compared to inactive patients, Riddell said.

The results showed that aerobic exercises such as walking, jogging or light cycling are associated with reductions in glycemia while anaerobic exercise like sprinting, heavy weight-lifting, and interval sports like hockey are known to temporarily increase glucose levels.

Hence, a clear understanding of the physiology of different forms of exercise and the changes that can influence glycemia during exercise may ensure safe and diabetes control, the researchers noted.

A research team from the university has found that by restoring the function of HSP72 — the main protein of HSR — improved glucose-related abnormalities.

The researchers developed a belt-type medical device that uses a special type of rubber.

“This device is very easy to use since it simply attaches to the abdomen and it has a low-impact on the patient. One can expect the effects to be similar to exercise therapy,” Tatsuya Kondo, who led the research, said in a statement.

The team then performed a clinical trial of MES + HS on 40 obese men suffering from type 2 diabetes.

Results showed a decrease fasting glucose levels, a loss of visceral fat, improve insulin resistance and a significant improvement in glycated hemoglobin (HbA1c) values.

“Even in patients who have difficulty exercising, such as those who are overweight, elderly, or have some form of disability, this device can be expected to provide acceptable treatment in addition to conventional diabetic medical care,” Tatsuya added.
